2024 Camp Forms

Choir Camp Forms

A complete electronic packet of camp forms is sent to each parent/guardian and camper for completion. 

  1. Read all rules and policies in the 2024 Choir Camp Handbook
  2. Complete the forms received via email (contact ChoirCamp@txstate.edu if you do not have these forms)

3. Complete the following OPTIONAL prescription medication forms

Guardian Authorization Waiver for Prescription Medicine (if camper will administer their own prescription medication during camp)
